Note to teacher/parents:
This survey should be administered BEFORE and AFTER students complete the CookShop program.
All responses will be private and the survey does not ask for information that can personally identify you or your student. The results of the survey will be used to assess the
effectiveness of the CookShop program- There are no “right” or “wrong” answers.
- Assist the student by reading each question and set of answers aloud two times. Pause after each potential answer option to provide the students with time to think.
- DO NOT assist the student by guiding them towards which answer to choose. It is extremely important that you DO NOT help or tell students which answers to give. Let them choose for themselves.
- Each student will choose their own best answer(s) for each question.
- For all questions, students should select the answer(s) they think are the most correct.
- Please complete the survey only once.
