This survey should be administered AFTER students complete the CookShop program.
All responses will be private and the survey does not ask for information that can personally identify you or your student. The results of the survey will be used to assess the
effectiveness of the CookShop program- There are no “right” or “wrong” answers.
- Each student will choose their own best answer(s) for each question. DO NOT assist the student by guiding them towards which answer to choose. It is extremely important that you DO NOT help or tell students which answers to give. Let them choose for themselves.
- For all questions, students should select the answer(s) they think are the most correct.
- Please complete the survey only once.
